For prospective parents or guardians, please review the above information. If you are interested in enrolling, please download our Registration Packet [PDF] and return completed forms to the school. Completed forms can be mailed, emailed, or dropped off at the school office.
In addition, parents will be expected to bring in birth certificates and provide up-to-date immunization records. We also ask you to complete the application for free and reduced meals [PDF] for our USDA school lunch program. Additionally, parents driving on field trips will be required to provide a valid driver’s license and proof of insurance.
Additional information for current and prospective parents
The state mandates that we share school immunization/exemption rates. This year (2017), our adjusted enrollment was 53 students, of which 37 claimed nonmedical exemptions for a total of 69.8%. In comparison, for January 2016, our adjusted enrollment was 39 students, of which 23 claimed non medical exemptions for a total of 59%. For January 2015, our adjusted enrollment was 48 students, of which 25 claimed non medical exemptions for a total of 52%.
